The blue line shows the intrathoracic pressure relative to the intraalveolar pressure (yellow line) that the patient needs to decrease in inspiration.

As you see in the middle graph where the patient has intrinsic PEEP (auto PEEP), the patient needs to exert more efforts in inspiration. This can be alleviated by adding external PEEP to decrease the gradient and the needed efforts by the patient (lower graph).
